The review noted the paucity of trial data to guide treatment choice in patients with heart disease, but in general, it suggested that tricyclic antidepressants are best avoided because of their pro-arrhythmic potential, whilst SSRIs appeared to be relatively free of unwanted cardiac effects. However, it stresses that the few trials of SSRIs and mirtazapine in patients with heart disease have generally been too small to exclude the possibility of such outcomes. It concludes nevertheless that “when drug treatment is needed for depression for patients with heart disease, SSRIs should probably be tried first.”
