South Africa Eyelid Surgery - The Procedure
During the surgery an extra skin, fat and muscle are removed from upper and lower eyelids. The surgery can repair a drooping upper eyelid and a "puffy" lower eyelid, which can make you appear tired or older than you actually are, they can also interrupt with your visual fields. You must know that this operation will not repair the wrinkles on the sides of the eyes, skin pigmentation around the eyes and fallen eyebrows. Eyelid surgery can be combined with face-lift, brow lift and forehead lift.

The operation usually lasts from one to three hours depend on the magnitude of the procedure. Usually the surgeon will fix the upper eyelid in both eyes and then the lower ones. During a standard procedure the surgeon will perform a skin excision in the folds of an eyelids. In the lower eyelids the incision is made through the eyelash line and it can go until the corner of the eyes. After the incision is made the surgeon will separate the skin from the fat and muscles beneath it, remove and extra tissue and put a gentle sutures. In case there is no extra skin present the surgeon will make and excision inside the eye so it won't be seen from the outside. This kind of procedure usually performed on younger patients with more elastic skin. The operation usually performed with local anesthetics and some sedatives given intravenous. NEI reaffirms its commitment to support ongoing research to better understand glaucoma, to identify risk factors that lead to the development of the disease, and to prevent vision loss and blindness.
  • Objectives  To use multiphoton microscopy to image collagen fibers and matrix structure in nonfixed human keloid tissue and normal human facial skin obtained following surgery and to compare the findings to existing knowledge of normal skin and keloid morphology to determine if this technology is a suitable adjunct for conventional histology. Methods  Epidermis was removed to expose the fibroblast-rich dermal layer that was then imaged using a multiphoton confocal microscope (Zeiss-Meta 510; Carl Zeiss, Jena, Germany). An 800-nm tunable titanium/sapphire femtosecond laser (Mai-Tai; Newport Co Spectra-Physics, Mountain View, California) was used to excite the tissue; second harmonic generation between 397 and 408 nm and autofluorescent signals were collected. Images were obtained using a Plan-Neofluar x40 oil immersion objective lens and a Plan-Apochromat x63 oil immersion lens. Results  Compared with normal skin, keloids showed disorganized collagen fibers arranged in complex swirls and bundles 20 to 30 µm in diameter. Normal tissue showed collagen fibers as distinct, straight strands less than 10 µm in diameter. Differences between normal and keloid tissue were subtle but apparent. Conclusions  The value of imaging living tissue is a significant benefit. Because keloids and hypertrophic scars result from altered collagen metabolism, the development of clinical multiphoton microscopy systems may allow examination of wound healing dynamics in vivo and potentially provides a means to monitor therapy without the need for biopsy or the risk of injury to tissue.
