The initial search located ten potentially relevant trials of which 7 (n=19,818) were eligible for the analysis. All had been published in full, and three had subsequent updates. On the primary outcome, there was an increased risk of 0.52% in the aromatase inhibitor arm compared to the tamoxifen arm; a risk ratio of 1.31 (95% CI, 1.07 to 1.60; P =0.007). Thromboembolic events were a secondary outcome, and these were less likely than in the tamoxifen arm (RR, 0.53; 95% CI, 0.42 to 0.65; P <0.0001). The authors conclude that treatment with aromatase inhibitors is associated with a small increase in risk of cardiovascular adverse events - while this is statistically significant, the number needed to harm is around 160 to 180.
