Canthoplasty in UA (Ukraine)  - What Is It?

Canthoplasty is a cosmetic procedure for reinforcing lower eyelid support. The lateral canthal tendon of the orbital bone is detached and a replacement is constructed. The aim of Canthoplasty is to strengthen the lateral canthus or the tissues that lie at the outer corner of eyelids and provide higher support to the lower eyelid. Sometimes, Canthoplasty might be done for reducing the surgical complications arising out of the incisions in the transcutaneous lower blepharoplasty.

Ukraine Visa requirements and customs

Tourist visas are no longer required for citizens of the European Union, USA, Canada, Japan, Switzerland, Liechtenstein, Andorra, Vatican, Monaco, Iceland, Norway, San Marino, Mongolia, Serbia, Montenegro and the countries of the Commonwealth of Independent States (except Turkmenistan). This applies only for tourist travel lasting less than 90 days.

Visas are required for travel related to study or work, even if for less than 90 days, even for the aforementioned countries. For citizens of other countries obtaining a visa for travel to Ukraine may take up to 10 working days; an express option is available at a higher cost. Always know how much currency you have with you. Customs officials will inquire about the amount being brought into the country. It is prohibited to bring large amounts of Ukrainian currency (hryvnia) unless it was declared upon leaving Ukraine.

It is advisable to check in advance the customs regulations (eg the Boryspol Airport website, which has an English version) as rules and regulations have the habit of changing at short and unannounced notice.

Canthoplasty in UA (Ukraine) - How Is It Done?

The Canthoplasty plastic surgery is done with a local anesthetic injection. When the anesthetic takes effect, the surgeon would make a cut into the lateral canthus in the direction of the orbital bone rim. The surgeon would isolate the lateral canthal tendon that extends from the bone rim to the lower eyelid. The attachment of the tendon to the bone would be cut. After that, a small incision would be made on the margin of the eyelid very near the cut tendon. All the adherent skin and conjunctiva in the tarsal plate would be cleaned. The strip of the tarsal plate tissue would be used for creating a new, tighter tendon. Sutures would be done to sew this strip to the lining of the bone inside the bony rim of the socket. Finally dissolving sutures would close the overlying skin and muscle.

Swelling in the lower and upper eyelids might persist for a few weeks or months. The instructions of the surgeon should be strictly adhered to avoid post-surgical complications. Canthoplasty is a complicated and delicate cosmetic procedure and the plastic surgeon should be an expert and experienced one for best results. If the plastic surgery is not properly done, the eyes might get an unnatural upper slant or downward retraction, particularly in persons with prominent eyes or in persons with weak cheekbone or orbital rim support.

A meta-analysis published in the Annals of Internal Medicine has concluded that N-acetylcysteine is the most effective agent for preventing contrast-induced nephropathy in patients with chronic renal insufficiency. However, whether this risk reduction translates into a benefit in clinical outcomes remains to be proven. Researchers conducted a meta-analysis to quantify the effects of individual strategies on the prevention of contrast-induced nephropathy and to facilitate the comparison of preventative effects across strategies. The meta-analysis included 41 trials in which treatment groups received either N-acetylcysteine, theophylline and other agents such as dopamine, fenoldopam, iloprost, statin, furosemide, trimetazidine, bicarbonate, ascorbic acid or mannitol. The primary outcome was the development of contrast-induced nephropathy, defined as an absolute increase in baseline creatinine greater than 44.2 micromol/L or a relative increase greater than 25% at 48 hours after contrast injection. The researchers reported the following results: • N-acetylcysteine decreased the risk for contrast-induced nephropathy compared with saline alone (relative risk 0.62, 95% CI 0.44 to 0.88) • The effects of theophylline on nephropathy were not statistically significant (0.49, 0.23 to 1.06) • Ascorbic acid reduced contrast-induced nephropathy (0.46, 0.23 to 0.90) • Bicarbonate reduced contrast-induced nephropathy (0.12, 0.02 to 0.95) • Furosemide increased the risk of contrast-induced nephropathy (3.27, 1.48 to 7.26) The researchers concluded that pre-procedural treatment with N-acetylcysteine and theophylline reduce the risk of contrast-induced nephropathy, but although theophylline reduces the risk, the reduction is not statistically significant. Additionally, the researchers state that fenoldopam, furosemide and mannitol did not produce beneficial effects. They recommend that the results of this meta-analysis should be evaluated in a head-to-head study to identify the most efficacious regimen for preventing contrast-induced nephropathy.
