Breast Reconstruction Procedure

This procedure is designed for women who underwent breast amputation after breast cancer or other conditions. Newer techniques allow the surgeon to create a breast which is very similar to the natural one. The reconstruction can be done during the amputation procedure, or a few months or even a year afterwards.

There are a number of implants: the most widely used are silicon implants which have silicon surface and filling. They come in different sizes and shapes.

Becker implants are made of a thick silicon surface (35%-50%) and a solution filling. The advantage of this implant is the ability to adjust its shape to the woman`s body. It can also serve as a skin stretcher. There are also implants with a thin silicon surface and a solution filling, which are less used.

The operation can be divided into 2 main categories: (a) a stretcher is inserted, then replaced with an implant; (b) the reconstruction is made from another body tissue.

As an island nation shut off from the rest of the world for a long time, Japan is very homogeneous, with around 98% of the population ethnically Japanese. The largest minority are Koreans, around 1 million strong, many in their 3rd or 4th generations. There are also sizable populations of Chinese, Filipinos and Brazilians. Indigenous ethnic minorities include the Ainu, driven north over the centuries and now found only on Hokkaido, numbering around 50,000 (although the number varies greatly depending on the exact definition used), and the Ryukyuan people of Okinawa.

The Japanese are well known for their politeness. Many Japanese are thrilled to have visitors to their country and they will be incredibly helpful to a foreigner looking lost and bewildered. On the other hand many are not used to dealing with foreigners — commonly known as gaijin (??, outsider), or gaikokujin (???, foreigner--a more polite phrasing) — and you may find them reserved and reluctant to communicate.



The most common reconstruction is performed with a stretcher and an implant. After amputation a balloon (stretcher) is inserted under the skin and chest muscles. It has a special valve which can be filled from outside. The implant is being filled gradually with a special solution, until the skin has sufficiently stretched for inserting the implant. It takes another 3-6 months until the breast has acquired its final shape, then the nipple can be reconstructed as well.

Becker implants: sometimes during amputation the skin can be preserved. That would eliminate the need for a stretcher, and a permanent implant (Becker) can be inserted right away.

The breast can also be reconstructed with body tissue from areas with extra skin and fat, like the tummy, buttocks and back. The tissue being removed is called "a flap". Common sites to acquire the flap are the lower tummy and the lotissimus dorsi muscle from the back.

Though treatment of secondary hyperparathyroidism with activated vitamin D analogues is linked to better survival in patients on dialysis., it is unknown if this is the case for patients with chronic kidney disease (CKD) not on dialysis. Therefore, researchers examined the link between oral calcitriol treatment, mortality and the incidence of dialysis in 520 male US veterans (mean age, 69.8 years; 23.5% black) with CKD stages 3 to 5, not yet receiving dialysis (mean estimated GFR 30.8 ml/min). Adjustments were made for age, race, comorbidities, smoking, BP, BMI, use of phosphate binders, estimated GFR, proteinuria, white blood cell count, percentage of lymphocytes, and levels of PTH, calcium, phosphorus, albumin, bicarbonate, and haemoglobin. In the study, 258 patients received calcitriol, 0.25 to 0.5mcg/d, for a median duration of 2.1 years. The incidence rate ratios for mortality and combined death and dialysis initiation were statistically significantly lower in treated vs. untreated patients (0.35; 95% CI, 0.23 to 0.54; p < 0.001 and 0.46; 0.35 to 0.61, respectively) in the fully adjusted models. These results were consistent across different subgroups. The researchers conclude that treatment of secondary hyperparathyroidism with calcitriol in patients with CKD stages 3 to 5 not yet on dialysis appears to be linked to statistically significantly greater survival. However, they acknowledge that RCTs are required to verify these observations and examine whether similar associations are seen with different activated vitamin D analogues. More...


This procedure can be complicated due to implant infection, a contraction of a surface of an implant, silicon leaks and anesthetic complications.

You`ll need to stay in hospital 2-7 days after the surgery, while you may feel pain and weakness for a week or two. Complete recovery may take up to 6 weeks, depending on the surgery. You may have an unusual sensation in the breast that would usually disappear over time. In most cases there could be a slight asymmetry between the breasts, however, most women report a significant improvement in their social life and self esteem.
