TW Breast Augmentation - The Plastic Surgery Procedure

The plastic surgery operation is usually done when the breast reaches its final size after puberty (ages 17-18). Except for cases when there is congenital breast hypoplasia or breast asymmetry in which it can be done earlier (local regulations in TW may be applicable).

There are three possible areas for the insertion of an implant for augmentation. The lower fold of the breast is the most common site. It allows full excess and lives a hidden scar. The second site is through the areola. This incision gives the best esthetic results but it is also the only one that goes through the breast tissue therefore the risk of infection and sensation loss is higher. The third one is through the armpits. Some plasit surgeons prefer it because the scar is almost invisible (except for those women who like to wear open close with no sleeves(. The disadvantage of this incision is a slight asymmetry of the breast. The decision usually made individually with each woman and the plastic surgeon (consult local TW surgeons).

The augmentation implant can be inserted above or below the chest muscles. The below position considered to be more preferable by most TW surgeons. There is less chance of feeling the implant, less chance of developing irregular folds or bumps on the surface of the breast and the implant is less likely to drop. This kind of plastic surgery technique is suitable for women with relatively little extra skin and less fallen breasts.

Most of the implants are made of silicon, and since the FDA determined that they safe for use they are being widely used by most plastic surgeons in TW.

The augmentation implants come in two shapes: round and pear shape also called "anatomic implants" because their shape resembles the natural contour of the breast. They are more expensive and require larger excision during the breast augmentation plastic surgery procedure.

The size of an implant usually depends on the woman size and proportions, the average size is 300cc. It is believed that smaller implants have lower complications.

The authors of this case report highlight the risks associated with using skin-lightening creams. They describe a case in which a 28 year old woman used skin lightening creams for 7 years. She presented with weight gain (12.7kg in 3 years), inability to conceive successfully, recurrent vulval candidiasis, bruising, pigmented striae on her body and a round face. An initial diagnosis of Cushing’s syndrome was made, however, blood results found low levels of cortisol and corticotrophin. After enquiring further about drugs, the patient told the clinicians about the cream she had been applying. The cream had been bought from a local shop not authorised to sell it and the patient was using 60g a week. Further investigations found the cream to contain clobetasol. The authors write, “Patients are often reluctant to admit that they have used skin-lightening creams, especially if these are supplied illegally. Similarly, doctors can be unaware of the need to inquire. But the market is worth millions of pounds a year, in the UK alone. Creams can contain toxic substances, such as steroids and hydroxyquinone - and patients are typically unaware of the risks”. More...
