Implant Removal Procedure

According to the American Society of Aesthetic Plastic Surgery:
235,000 women had breast augmentation surgery in 2003;
More than 40,000 (1 every 6) had breast implant removal surgeries.

This procedure, known as explantation, can be done to decrease the size or to make room for new implants.


Reasons for Breast Implant Removal
The three most common reasons for breast implant removal are:
• Change of size and shape;
• Implant leakage or rupture; and
• Capsular contracture.

Sometimes, breast implant removal is necessary to treat problems that occur with the implants.

Possible problems with implants
• Deflation or rupture
• Shifting
• Wrinkling
• Sagging
• Asymmetry

Problems due to the body's reaction to the implants
• Bleeding
• Infection
• Diagnosis of breast cancer
• Formation of scar tissue that tightens around the implant (capsular contracture)
• Necrosis, or the formation of dead tissue around the implant, which may prevent wound healing
• Calcium deposits


The procedure
Breast implant removal is done under either general anesthesia or local anesthesia combined with sedation.
The procedure usually takes 30 minutes to an hour and takes place in an outpatient surgical center.
The surgeon removes the implant by either:
• Operating through an incision under the breast; or
• Incision through the nipple.
If the implant is made of saline, doctors may choose to deflate it first to facilitate removal.

Average costs
Breast implant removal usually costs between $1,000 and $4,000.

Recovery
Initial recovery from breast implant removal surgery is usually quick, with many patients returning to everyday activity within a few days, and full activity within two to three weeks.
Full recovery often takes a few months. For the first few days after breast implant removal, there may be mild discomfort, swelling and bruising.

Complications
Some uncommon (though possible) complications include:
• Loss of nipple sensation;
• Scarring;
• Bleeding; and
• Loose skin.

Removal of Large Breast Implants
Occasionally, women who opt for removal of large breast implants, especially those that are inserted on top of the muscle and under the breast glands, are left with major cosmetic deformity in their breasts if they opt to not replace the breast implants or have further cosmetic surgery.


Tissue atrophy and chest wall deformity often result from breast implant removal. Breasts may also be smaller than they were before the implants because of hormone changes or weight loss. Sagging may also become more apparent after breast implant removal.

Emotional Effects
Some women suffer from psychological distress after breast implant removal.
It is normal to feel some sadness.


If psychological symptoms last a reasonable amount of time after breast implant removal, the patient should seek professional help.

  • Portugal Climate

    Portugal is one of the warmest European countries. In mainland Portugal, yearly temperature averages are about 15°C (55°F) in the north and 18°C (64°F) in the south. Madeira and Azores have a narrower temperature range as expected given their insularity, with the former having low precipitation in most of the archipelago and the latter being wet and rainy. Spring and Summer months are usually sunny and temperature maximum are very high during July and August, with maximums averaging between 35°C and 40°C (86°F - 95°F) in the interior of the country, 30°C and 35°C in the north, and occasionally reaching 45°C (113°F) in the south. Autumn and Winter are typically rainy and windy, yet sunny days are not rare either. Temperatures rarely fall below 5°C (41°F) nearer to the sea, averaging 10°C (50°F), but can reach several degrees below 0°C (32°F) further inland. Snow is common in the mountainous areas of the north, especially in Serra da Estrela. Portugal's climate can be classified as Mediterranean (particularly the Algarve and Alentejo, though technically on Atlantic shore).



  • Portugal Talk

    The official language of Portugal is Portuguese. Although it's somewhat related to Spanish, Italian, and other Romantic languages, it's not identical. Moreover, pronunciation of some Portuguese letters and words can be daunting. Spanish-speakers will be able to make themselves understood: if you choose to speak Spanish (or have no alternative), try to speak slowly and evenly. Your chances of being understood that way are quite high, but don't expect the locals to speak or understand Spanish.

    English is spoken in many tourist areas, but is far from ubiquitous. However, the younger Portuguese will speak at least some English, or French.

    The Portuguese people are of generally excellent humor when they are interested in talking with someone who can't speak their language. This means that all manner of shop owners, sales-folk, and people curious about you will take time to try to carve out any means of communication, often with funny and unexpected results. If the traveler attempts to speak Portuguese with locals, the action is taken with respect and oftentimes the local will apologize for how "difficult" it is to learn Portuguese, or how "hard" the language is. This good favor might encourage travelers to learn the very basics of Portuguese, such as daily greetings and the routine "please-thank you" exchanges.

  • Abstract Background  Adjustable breast implants are widely used for both reconstructive and cosmetic breast surgery. They provide unique postoperative versatility and allow for more effective management of numerous conditions that would otherwise require surgical intervention. Findings have shown that in a clinical setting, it often is necessary to overexpand saline implants beyond the manufacturer’s recommended fill volumes for positive results and optimal patient satisfaction. The authors investigated their breast-augmentation patients, comparing implants expanded beyond the manufacturer’s fill volume with implants that remained within the recommended parameters. Methods  A total of 138 patients (270 implants) undergoing breast augmentation mammoplasty with Smooth Round Spectrum implants were evaluated postoperatively. To determine the effects of overexpansion, the incidence of leakage was assessed as well as the possibility of increased firmness. Patients completed a satisfaction questionnaire. Results  The findings showed no evidence of increased leakage with implants expanded beyond recommended fill volumes, and 97% of the patients reported a perfect score for satisfaction ratings. Of the 270 implants evaluated in the study, only 7 were given a less than perfect score. Conclusions  By exceeding the recommended fill volume, the authors were able to use the implant to its maximal potential, with increased patient satisfaction. The authors were able to correct problems and avoid complications that could not have been resolved without deviating from the manufacturer’s recommended fill volumes.
