Polynesians migrated to, and established communities on, the islands of Hawaii before the arrival of Captain James Cook in 1778, who is widely credited as the first European visitor to the islands. At that time, each island was a separate kingdom. With the support of western advisors and weapons, Kamehameha I of the island of Hawaii conquered all the islands except Kauai, which acquiesced to his rule in 1810.

After Kamehameha II abolished the kapu (taboo) system, American missionaries came to the islands to spread Christianity. Their children would later become successful businessmen in the Islands. Pineapple and sugar cane plantations were established, and workers from other countries were imported as contract laborers. Later, their descendants would also become established as successful professionals.

The Hawaiian monarchy was overthrown in 1893 by a group of American businessmen. While the U.S. administration at the time refused to annex the former sovereign nation, in 1898 the United States did annex the islands, which became a territory in 1900, and a state of the United States in 1959.

The calf implant procedure can take up to 2 hours per implant to complete. A general anesthetic is recommended together with an overnight stay in the hospital. During the calf implant procedure, an incision is made in the natural crease behind the knee. The surgeon then creates a pocket large enough to place the calf implants. Silicone semisolid implants are planted in the calf of each leg. The calf implants can be placed on both the upper inside or outside of the leg. This area, consisting of muscle and fat, will form scar tissue that will help keep the implant in place. The incision is closed with sutures, and a bandage is applied to reduce swelling and discomfort.

All surgical procedures carry an inherent risk of certain complications. Risks can include infection, blood loss, fluid collection, movement of the implant or scarring. There is also a risk of adverse reaction to anesthesia. Significant complications from calf implants are unusual, however. In extremely rare instances, the calf implants may weaken the nearby muscle. If the implant is infected, ruptures or moves, the implant may have to be removed, which is not always a straightforward procedure. Abstract  Surgical management of facial hemangiomas, the most common childhood neoplasms and infiltrating facial vascular malformations, has the risk of injury to the facial nerve and remains controversial. The aim of this study was to analyze the use of the commercially available Neurosign 100® Nerve Monitor in those cases of facial vascular anomalies where resection was considered the appropriate treatment. On a prospective basis, six consecutive patients with deep (so-called infiltrating) vascular anomalies of the face (two hemangiomas, three lymphatic malformation, one lymphatico-venous malformation) were enrolled in this study. During surgical preparation, electromyographic (EMG) activity of the muscles innervated by the facial nerve was monitored. The variables collected included the patient’s sex, age at surgery, location of the lesion and depth, prior treatment, diagnostic workup, site of skin incision, operating time, operative and postoperative complications, and reconstructive outcome. All lesions were resected without major bleeding. Postoperatively, all patients showed regular, symmetrical function of the facial nerve. The mean operating time was 136.7 min (range 90 to 240 min). During the preparation using surgical instruments, EMG activity was noted both as a graphic signal showing facial nerve activity and as an acoustic signal. In all patients, the branches of the facial nerve were identified clinically by electrode stimulation at 30 Hz. Postoperatively, all patients had improvement of facial asymmetry and deformity; preoperative pain episodes and functional impairments resolved completely. Intraoperative facial nerve monitoring provides feedback regarding the location, extent, and ongoing functional status of the facial nerve in the surgical management of infiltrating benign lesions such as hemangiomas and vascular malformations. Besides an overall distinct indication for surgery, we recommend its use for prevention of iatrogenic facial nerve injury.
