Miltefosine has been recently approved for the treatment of visceral Leishmaniasis in India and Germany, but (to the authors’ knowledge) has not yet been used in transplant patients. A few reports describe the successful use of ketoconazole plus allopurinol or fluconazole plus allopurinol; however allopurinol may increase the concentration of azathioprine and this should be borne in mind.
